It is presumed that the specimen or sample (s) belong to the customer or patient named are identified in the test requisition from the reported results are restricted to the given specimen or sample (s) only.

Laboratory test results may vary from lab to lab from time to time for the same parameter for the same patient. The reported results are dependent on patient preparation / individual assay methods / equipment used.

Neither icon diagnostics not its directors / employees / representatives assume any liability responsibility for any loss / damage that may be incurred yo any person as a result of persuming the meaning or content of the report.

Icon diagnostics will not be liable for any decision taken by any appropriate medical expert on any treatment based on the report.

Reconfirmation shall be sought if the reported results be unexpected or do not correlate clinically. A repeat investigation may be entertained within 48 hours.

Any queries regarding the authenticity of the test results should be verified within 2 days form the date of sample collection on request by the same referring clinician.

All the specimens or samples collected are required to be prepared, stored, labelled and brought as per the instructions of icon diagnostics. icon diagnostics cannot be liable for incorrect results of any sample which are not maintained as per the issued instruction.

Partial reproduction of the report is not permitted / valid

All dispute / claims concerning to this report are subject to the jurisdiction of court of hyderabad telangana, India.
